The Continuum of Care started in Primary Care continues through all three stages of care. In Continuing Care, a discharge plan is formulated to maximize conditions for sobriety. Each patient agrees to a specific plan with his or her counselor.

The Continuum of Care started in Primary Care continues through all three stages of care. In Continuing Care, a discharge plan is formulated to maximize conditions for sobriety. Each patient agrees to a specific plan with his or her counselor.Plans could consist of:
- Ongoing Alcoholics / Narcotics Anonymous meetings, assisted by contacts made while at Maple Leaf Farm. The plan encourages 12 step involvement and individual program sponsorship.
- Attendance at weekly Maple Leaf Farm continuing aftercare meetings in Chittenden county.
- Other resources as appropriate:
- Intensive Outpatient Program (IOP).
- Family counseling.
- Medical, spiritual and / or psychological consultation.
- Employment / Vocational related training.
